Paper blocks will be constructed for children and teens who were permanently damaged by a firearm prior to their 20th birthday.

To create a paper block we need:

• a picture of the child (send an extra copy or color xerox - a lage picture or 3 or 4 clear snapshots could be used depending on other information put on it.)
• date of birth
• date of injury
• information about the child/teen before the injury
• information about how the child/teen was injured by a firearm (accident, relative, another child, drive-by, suicide atempt, etc.)
• information about life for the child/teen after the injury
• City & State
• Also include your name, address, telephone number, and relationship to the child.
